What are the most effective treatments for scoliosis?
Scoliosis is very different from back pain or lower back discomfort, which would go away normally and even without medication or physical therapy treatments. The disease tends to progress, especially in the rapid growth phase.
When left untreated, scoliosis can lead to severe deformities of the trunk, limit the functional biomechanics of the chest, ability to exercise, general fitness and ability to work, and several other factors that are related to compromised quality of life.
In most cases, scoliosis is discovered by the family and in the face of this, many do not know how to proceed and even which doctor to look for or which treatment is ideal. In Brazil, the main treatments indicated by orthopedic doctors who are not specialized in the area are the techniques of:
- “Wait and see”, analyzing the case every six months;
- Do swimming;
- Pilates and RPG;
- The use of the Boston or Milwaukee vest;
- Surgery.
But, these treatments are not always the best alternatives. Therefore, focusing on specific treatments for idiopathic scoliosis and non-idiopathic scoliosis becomes essential to not cause major health problems for women and men.
How to be successful in treating scoliosis?
In recent decades, “wait and see” has been replaced by indications of:
- Note – for cases from 0 to 10 degrees;
- Exercise treatment – for 10 to 25 degrees;
- Treatment with vest and exercises – from 25 degrees to 45 degrees;
- Surgical treatments – for curvatures greater than 45 and 50 degrees.
To be more successful in the treatment, it is necessary to evaluate each case to follow the levels of progression and how they can be avoided with each type of exercise based on scientific evidence, use of modern vests, and in last cases, and very delicate processes, resort to surgery.
Treatment: observation
The observation is to evaluate cases of curvature from 0 to 10 degrees, with the Adams test; use of Bunnel scoliometer and radiography. You need this path to be sure what the next steps will be for your patient’s treatment.
Treatment: exercise methods based on scientific evidence
Some specific exercises bring very positive results in scoliosis treatments. Especially in cases of 10 to 25 degrees. These methods are proven by PUBMED and SOSORT and include the German Schroth method, BSPTS, DOBOMED and Side Shift methods and also the Fits, Lyon and Isisco methods.
With these evidence-based exercises, many cases have grade regression and even less decrease the chances of vest use by teenagers.
Treatment: the types of vests and the most indicated
When should I indicate the vest to my patient? Which vests get the most results? In Brazil, two types of vests are highly recommended, such as the Milwaukee vest and the Boston vest, which require a long time of use and most teenagers do not like to wear them at school due to their appearance. Thus, the rate of improvement is greatly reduced among patients.
In the world, there are several other types of vests that are three-dimensional, which are more efficient and reduce the indication for surgery. These vests, from the Goss System, are made by scanner that circulate the patient and have extremely personalized models for each patient’s case.
Surgical treatment
Surgical treatment is indicated in cases of 45 or 50 degrees. During the waiting period for surgery, previous treatments are used to lessen patient complaints.
