LOOK FOR GIBBOSITY
It is important for the doctor to systematically carry out a complete examination of the spine on an undressed child, barefoot. We measure your height standing and sitting. The sitting height gives an idea of the remaining residual growth on the spine.
For information, for children of normal height, the sitting height at the end of growth will be 88 cm for a girl and 92 cm for a boy. We can also help each other to check the potential for growth, using the usual charts. The sports doctor must carry out this examination either on hard ground or on a podoscope. He will take the opportunity to check for plantar static disorders and alignment disorders of the lower limbs. A plumb line examination is essential.
We find a:
- Pelvic tilt
- Hyperlordosis
- Chest asymmetry
- Chest hump or hump+++
At this stage of the examination, it is important to evaluate a differential diagnosis between scoliosis and scoliotic attitude. In all cases, the humpback is absent in the case of scoliotic attitude.
To check this humpness properly, the child is asked to lean forward. In addition, we take the opportunity to note any constraints and musculotendinous stiffness.
Management of idiopathic scoliosis
- Regular monitoring
- Re-education
- Corset
- Postural education standing and sitting
- Physical activity
Radiological diagnosis
Its purpose is to confirm the existence of scoliosis by measuring the Cobb angle. It also allows to calculate the value of vertebral rotations. Finally, it allows to assess bone maturation with the realization of a Risser test. This follow-up is a capital point which appreciates the evolutionary factors.
The results will be as follows: strict procedure
- Total spine from the front and in profile on large film, with possible correction of inequalities in the lower limbs.
- Basin for Risser test.
- Possibly, exploration of bone age.
- Lying shot if necessary.
Attention: we will look for the existence of a rotation to speak of real scoliosis. However, it is necessary not to irradiate the children too much and to ask for the right examinations allowing the implementation of a treatment and an adapted follow-up.
