Techniques for breast augmentation

Breast augmentation can be done with implants or by fat transfer.

Breast implants come in saline and silicone varieties, as well as many sizes, shapes, and textures. The implants can be placed above, below, or in a mid-position of the pectoralis muscle, through a series of incision locations.

  • Subglandular: In the subglandular anatomical position, the breast prosthesis is located immediately below the mammary gland, with intimate contact between the prosthesis and the glandular tissue; In this case, below the prosthesis is the pectoral muscle with its included fascia (membrane that covers it).
  • Submuscular: In the subpectoral or submuscular position, the breast implant is placed below the pectoral muscle, partially detaching said muscle in the lower part to prevent muscle contraction from moving the muscle upwards (dynamic breast). This placement is also called Dual Plane since most of the prosthesis is covered by the pectoralis major muscle, but a small part is in direct contact with the mammary gland.
  • Dual Plane: In the subfascial plane or position, the prosthesis is positioned in a virtual space created between the pectoral muscle and its fascia, which is a kind of thin covering that covers it. In this case, the pectoral muscle is below the implant and the muscular fascia and the gland are above it.

The subfascial plane is an evolution of the subglandular plane that does not provide enough tissue to achieve naturalness in thin patients, nor is it as effective an isolation between the prosthesis and the gland as the submuscular plane in terms of reducing the incidence of capsular contracture.

Fat transfer, or fat grafting, involves harvesting fat cells from one area of ​​the body, commonly the stomach, hips, or thighs, and injecting them into another. The fat is removed from the body by liposuction, processed and purified by centrifugation, and injected into the breasts. Fat is reabsorbed up to 30% depending on each patient.

An article from the National Center for Biotechnology Information indicates that if the volume of the transplanted fat in the breast is less than 150 ml on each side, the result of the increase is less obvious. Therefore, in our opinion, autologous fat transfer for breast augmentation is not suitable for thin women who do not have enough fat to deposit.

How to choose the correct size for my breast augmentation?

Breast implants and fat used for fat transfer are measured in cubic centimeters (cc). The same implant size or amount of fat will look different on each person. It is important to discuss expectations and desired results with your surgeon so that he or she can help you decide what is right for you. Many doctors use digital imaging software as a way to see what it would look like with different cc levels. Another option to check your next size is to try on a trial prosthetic bra.

What is the profile and projection of the implant and why is it so important?

The naturalness and feel of a larger or smaller prosthesis is largely determined by the projection of the implant. The projection of the implant is the measure of how much the implant protrudes outward, that is, the degree to which the breast will protrude from the chest. To measure the profile or projection, the needs and tastes of the patient must be taken into account. Normally there are 3 or 4 types for each prosthesis and the high profile (3/4) or the extra high (4/4) are generally used, and the medium profile (2/4) in those women who seek more natural results.

The higher the profile, the bigger the chest will look. Keep in mind that the projection of the implant is added to what you already have before surgery. If you don’t have any bust, opt for high or extra high projections. An implant with a medium projection of 350 grams is wider, higher and less projected than another with a high projection of the same size.
