Scoliosis

The affected vertebrae undergo translation and lateral tilt, accompanied by rotation. Consequently, scoliosis is generally accompanied by shortening and deformation of the trunk.

In most cases, a structural scoliosis initially manifests as functional scoliotic variations. Over time, these functional deviations progressively turn into structural deformations.

There are different types of scoliosis depending on the age of onset, the shape of the curves or the assessment standard applied.

  • Childhood idiopathic scoliosis
    (Appearance between 0-3 years of age)
  • Juvenile idiopathic scoliosis
    (Appearance between 4-9 years old)
  • Adolescent idiopathic scoliosis
    (Appearance from 10 years of age)
  • Lumbar scoliosis
  • Thoracic scoliosis
  • Scoliosis of 3 curves
  • Scoliosis of 4 curves

Epidemiology

Idiopathic scoliosis usually manifests in adolescence and has a tendency to progress until the end of physical growth. Most scoliosis is detected between the ages of 10 and 12. But the deformation begins between 5 and 8 years.

Between 2 and 4% of the population suffer from scoliosis with a curve of more than 10 degrees. A curve of less than 10º is not considered scoliosis. Not all deviations of more than 10 degrees are considered scoliosis. It may be scoliotic attitudes.

The disease has a higher incidence in females (4:1).

The origin and evolution of idiopathic scoliosis is unknown. Since scoliosis is usually painless, it is rarely diagnosed early. However, early diagnosis is important, since the earlier it is diagnosed, the better the prognosis.

Signs and symptoms

Generally, patients with scoliosis do not have very intense discomfort in the initial phase. It may happen that they suffer from back pain when doing physical work, when playing sports or when adopting sustained postures.

In most cases, the first time scoliosis is detected is by observation of the patient himself or of a family member or friend. Some type of symmetry is detected that induces the subsequent diagnosis.

We recommend all parents to observe the physical appearance of their sons and daughters in order to detect possible external signs of scoliosis:

  • Uneven shoulders
  • Uneven hips
  • Asymmetrical triangles between trunk/pelvis and hanging arms
  • Scapula prominent and to the side of the greater convexity
  • Thoracic hump

CHARACTERISTICS OF THE TREATMENT

ADAMS TEST

It is a very easy test to perform, even by people without health training.

The patient stands facing forward, with the feet together and flexes the trunk forward, keeping the legs straight and making the gesture of touching the feet.

A column without deviations will flex symmetrically. The dorsal aspect on both sides of the spine will be level.

A scoliotic spine will flex asymmetrically. Due to the rotation of the vertebral bodies, the ribs attached to these vertebrae will move dorsally (back) on one side and ventrally (front) on the other side. Consequently, the dorsal aspect on one side of the spine will be higher than on the other side.

One of the most affected joints, both in people who practice sports and in those who do not, are the knees.

The generalized reaction is usually rest. This decision is not always the best, as it has been shown that most knee ailments are due to a lack of muscle strength. The muscles are too weak to cope with the demands of the joint.

Therefore, although it is correct not to force the joint in the acute phase, it is essential to strengthen the muscles, so that it can support the load of the body weight, fulfill its dynamic and stabilizing function and, ultimately, prevent relapses.

Prior to starting treatment, we carry out a first consultation in which we explore the patient, evaluate their clinical history, their diagnostic tests and determine the indication for treatment.

Using radiography, the type of scoliosis and the degrees of deviation can be determined with total accuracy.
