What is gynecomastia surgery?

Hormonal imbalance, aging or taking certain medications are the main causes of gynecomastia. All men have a breast bud that remains little or not visible. In the case of gynecomastia, the mammary gland is swollen and forms a firm protrusion around the areola.

There are many causes of gynecomastia:

  • Certain cancers, such as cancer of the testicle especially, but also the kidney or the lung which secrete female hormones.
  • Pathologies of the endocrine glands, such as hyperthyroidism or prolactin adenoma are the most common.
  • Certain medications, such as anabolic steroids found in bodybuilders, but also estrogens (used by transgender people for example to have breasts).
  • Liver diseases, such as hepatitis, cirrhosis or hepatocellular insufficiency.
  • Alcoholism.

To radically correct the hypertrophy of the chest, one can opt for gynecomastia surgery. The procedure involves removing excess breast tissue. The removal is permanent and in the long term, recurrences are rare.

Indications: Why and When to correct gynecomastia?

Unlike adipomastia or fatty gynecomastia, glandular gynecomastia cannot be eliminated by sports and bodybuilding. Although having no serious impact on health, it is above all a source of psychological discomfort preventing some men from being comfortable in their bodies. The idea of ​​having “women’s breasts” can be a big complex.

Sometimes gynecomastia causes pain. Surgery thus provides an aesthetic solution and psychological relief.

In adolescence, the appearance of pubertal gynecomastia is possible. This form of gynecomastia is normal and normally goes away when hormonal fluctuations have stabilized. It usually fades after 17 to 18 years. In most cases, gynecomastia therefore occurs in adulthood.

How is the correction of gynecomastia carried out?

Before surgery for gynecomastia

The first palpation examination already allows to know if it is a simple excess of fat or a glandular gynecomastia. A more in-depth diagnosis and a complete assessment are however necessary to clearly identify the problem. To rule out the possibility of cancer and check the composition of the protuberance, a mammogram is performed before considering gynecomastia surgery.

Once the hypertrophy by increase of the mammary gland is proven, endocrine and biological examinations complete the diagnosis. The goal is to determine the cause of the problem (hormonal imbalance or drug cause).

Here are the main actions of the review

  • An ultrasound and palpation of the testicles to detect the presence of an abnormal nodule in cause of a possible excessive production of hormones.
  • A biological assessment to check, among other things, the level of creatinine, the dosage of prolactinemia, testosterone, LH (luteinizing hormone which stimulates the production of progesterone) and FSH (or follicle-stimulating hormone stimulating the synthesis of estrogen).
  • A liver test to detect malfunctions in the metabolism of thyroid hormones.

These tests can be ordered by the surgeon or the patient’s doctor.

Once the hormonal and endocrine assessment has been completed in addition to the renal and hepatic assessments, the gynecomastia correction technique is explained to the patient. The decision for surgery is preceded by a period of reflection of 15 days after the first consultation.

The interview with the surgeon requires at least one or two preparatory visits. This is an opportunity to discuss the course of the intervention, the risks to be taken into account and the habits to adopt after the operation. An appointment with the anesthesiologist should also be scheduled within 2 days before the operation.

During surgery for gynecomastia

This surgery is most often done on an outpatient basis. A small incision is made at the level of the lower hemi-circumference of the areola. As the mammary gland is fibrous and thick, liposuction cannot eliminate it. We therefore proceed to ablation to remove the excess mammary gland. The latter should always be sent to a pathology laboratory for analysis.

Liposuction is reserved for the removal of fatty deposits. The subject may have both excess mammary gland and surrounding fat. In this case, the ablation will be followed by liposuction. In case of ptosis and excess skin, a breast lift is performed to remove the excess skin and have a better result.

After gynecomastia surgery

During hospitalization, the patient keeps a compression bandage around the chest to limit the risk of hematoma. The pain is a little more severe compared to liposuction surgery, but it is well treated with conventional painkillers. The operated area must be regularly cleaned a few days after the intervention.

The convalescence is generally one week, sometimes two. Professional activity can be resumed afterwards, and sports activities can resume after 4 weeks.

The results of surgery for gynecomastia

The final result is assessed after 2 months. It is necessary to wait for the edema to subside and for the skin to perfectly match the shape of the pectoral muscle. As for the scars, they will be discreet, because they are hidden around the areola.

The optimal result is obtained once healing is complete. The speed of healing depends on each person.

The duration of surgery for gynecomastia

The duration of the operation for removal of the mammary gland is one hour on average, compared to 30 to 45 minutes for liposuction in the event of adipomastia.
