Who is a candidate for endoscopic brow lift?
Many patients undergoing upper blepharoplasty have some degree of drooping of their eyebrows. Most of these patients will have a satisfactory outcome simply with a blepharoplasty, as they show mild degrees of brow droop. However, in moderate or severe patients, blepharoplasty alone will not clean the eyelid sufficiently, so brow lift is needed.
The most frequent candidates are women over the age of 40 who want rejuvenation of the upper third of the face and are in good general health.
What is endoscopic brow lift?
Brow lift surgery consists of raising the forehead and eyebrows to the desired position. To do this, we must weaken the ligaments and muscles that hold the eyebrows in place.
Access is through 5 small 1.5 cm incisions in the scalp so they are not visible. Through these, with the help of a small camera (endoscope) and a tool, the suspensory ligaments of the eyebrows and the depressing muscles of the eyebrows are reached subcutaneously. After weakening, the forehead and eyebrows are now free and can be preset to the desired height. This can be done in different ways depending on the situation, but basically we use self-absorbing fixation devices or sutures.
It is common to have an upper blepharoplasty combined with a brow lift, but in these cases blepharoplasty is always more conservative. If necessary, a lower blepharoplasty can also be performed in the same surgical action.
Other techniques to raise eyebrows
Sometimes we choose another technique to raise eyebrows because of the patient’s characteristics. For example, in cases where only the tail of the eyebrow is lowered, we can only make two incisions on the scalp by choosing to perform a facelift only on that part of the eyebrow.
In older patients or those with facial paralysis, we generally prefer external eyebrow lift (direct eyebrow aesthetics). This technique is performed with an incision made just above the hair follicles of the eyebrow, so if done well it is usually not noticed within a few weeks.
Recovery after brow lift
In the post-brow lift period, discomfort and tension are usually observed, especially at the temple level. These symptoms improve with anti-inflammatory drugs and usually subside within the first week. Otherwise, recovery is quick. When the surgery is performed alone, bruises are usually not seen, but some swelling is seen in the area. Recovery is not complete after one week, but most patients will have no problem resuming their ‘social’ activities at this point.
In the first days, it is important not to exert vigorous physical exertion to prevent possible bleeding and shorten healing. We recommend frequent cold application for the first 2-3 days, oral anti-inflammatories and antibiotic ointment for the cuts.
