HOW TO KNOW IF THE DROP IS EXCESSIVE

One of the most frequent consultations for dermatologists is hair loss. The first thing that the specialist assesses is if we are facing a physiological hair loss (healthy, so to speak) or if it is something pathological, that is, if it is a symptom of some disease or disorder.

HOW THE DERMATOLOGIST MEASURES IT

There are several maneuvers, commonly used by doctors, to find out if hair loss is excessive or if, on the contrary, it is normal hair loss:

  • Sabouraud’s maneuver or plucking sign: It consists of pulling the hair from different areas of the scalp (frontal, parietal, occipital and crown or vertex areas). If from each strand of 10 hairs we pull out more than one, the maneuver is positive. In other words, we would consider the fall to be excessive and we would have to study what causes it.
  • Count sign: Requires a little more patience. The patient is asked to wash his head two days in a row. The hairs that have been detached after combing are counted. If there are more than 80, it is considered a positive test. To finish defining it, it is necessary to assess which territories it affects, and if it is something reversible or not.

In the event of excessive hair loss, to rule out secondary causes, an analysis is usually carried out.

  • It assesses the possible existence of anemia or a thyroid problem (frequent causes of alopecia). And depending on the patient, and what he relates, some other test is performed to rule out other reasons.

TYPES OF ALOPECIA AND THEIR CAUSES

  • When it is genetic, it is called androgenetic alopecia and can be due to both hereditary and hormonal factors. It usually starts early, at the age of 20-30, the hair becomes thinner and scarcer and the fall is noticeable especially on the top of the head.
  • We speak of universal alopecia when it does not affect a defined area, but rather any part of the scalp.
  • On the contrary, alopecia areata is the one that is located in a single area (or several, but always isolated, not contiguous to each other).

TOO MANY ANDROGENS

Diffuse universal alopecia can be due to several reasons, but it is usually due to an excess of male hormones and occurs in two patterns, depending on whether it affects a woman or a man.

  • Thus, in men, it follows the Hamilton pattern, initially affecting the frontotemporal area and the crown.
  • In women, however, it follows the so-called Ludwig pattern: the upper area of ​​the scalp is progressively lightened, without affecting the implantation line (the frontal area). Although it is important to note that cases of frontal alopecia in women are increasing.
